Community Health Social Work Service
Community Health Social Workers offer a safe environment to talk through and problem-solve concerns. Services are for 18yrs + and include:
- support through grief, loss and bereavement,
- support to access financial assistance and housing,
- group work,
- health promotion, and
- working with community.
The Community Health Social Worker can help to access services and personal and community resources for health and well- being. You may self refer, or be referred by your GP or other service providers.
The service is confidential and there is no charge.
Usual days are Tuesdays & Wednesdays and alternate Mondays.
The Community Health Social Work service is co-located with the Scottsdale Doctors Surgery and is provided through the Tasmanian Health Service.
